- Complimentary consultation requests. When you submit the consultation request form, we collect your full name, email address, an optional phone number, and an optional message. We do not ask for, and you should not submit, medical histories, diagnoses, or other sensitive health information through this form.
- Speaking and media inquiries. When you submit a speaking or media inquiry, we collect your full name, organization, email address, optional phone number, engagement type, event name, event date, event location, estimated audience size, proposed topic, budget, and any additional details you choose to provide.

2.3 Information collected by third-party scheduling
If you use the Carepatron scheduling link to arrange a consultation, you will leave this website and any information you provide to Carepatron is governed by Carepatron’s own privacy policy and terms. We do not control how Carepatron collects, stores, or uses your information.

Form submissions (consultation requests, speaking inquiries, and newsletter subscriptions) are stored so that Institute staff can review and respond to them. Newsletter subscriber records are stored with a record of your consent.
Membership account data is stored in the same hosted database. Subscription billing and payment processing are handled by the Hostinger Online Store and its payment processor; payment card details are not stored by the Institute.

Once you follow an external link, your activity on that third-party service is subject to its own privacy policy and terms. We encourage you to review the policies of any third-party service you use.
7. Data Retention
We retain form submissions and account information for as long as needed to provide our services and respond to inquiries, and thereafter as needed to comply with legal or operational obligations. You may request deletion of your newsletter subscription at any time using the unsubscribe page.
8. Your Privacy Rights
Depending on your location, you may have rights to access, correct, or delete your personal information, and to withdraw consent for processing based on consent. To exercise these rights, contact us using the details in Section 1. We will respond within a reasonable timeframe.
